If your child has not previously booked into the MELC Vacation Care program, please provide the following details so we can determine your entitlements:

I give permission for my child to participate in all activities and understand they will be transported by bus.

I understand that the excursion information sheets for the days I have selected will be emailed to me on receipt of this authorisation.

I am aware that the risk assessments have been conducted for each activity and are available on request.
